This is WAY beyond what Muse is capable of and not really what Muse was designed for.
This requires the creation of a database on your server and code to handle it and serve the correct content. This is also a security minefield - serving pages with user input and data processing is a whole different game to serving static preconfigured pages.

My recommendation: 1) hire a developer to write you this and to run you through hosting it on your server and then just link that page in Muse
2) OR do something yourself with google docs. Spend a day writing an excel spreadsheet that does what you want (certainly achievable) and then make sheet look as professional as you can. Host online with something like: Forget Excel: 14 Online Spreadsheet Applications . Redirect to spreadsheet from Muse
